To register for a SCORE workshop other than on-line, print and fill out this form, make a copy for your records, and submit the form with a check made payable to “SCORE Chapter 78” at the door or mail it to the following address no later than one week prior to the scheduled date:

Occasionally it is necessary to cancel a workshop due to inclement weather, insufficient confirmed registration, or other reasons. In such cases, a cancellation notice will be posted on this page and we will attempt to notify all registrants and refund registration fees. We therefore recommend registration at least one week prior to the scheduled date of the workshop. Please check this Webpage on the morning your workshop is scheduled for any cancellation notices.
